Job Description

Join our team as a Registered Nurse (RN) in Long-Term Care (LTC) and bring comfort, compassion, and expert care to those who need it most. In this role, you’ll build meaningful connections with residents while ensuring their medical and personal needs are met in a safe, supportive environment where dignity and quality of life come first.

Responsibilities:
• Closely observe residents and document any physical or emotional changes, communicating important updates to the care team.
• Support residents with personal care needs and daily activities, always delivering respectful and compassionate assistance.
• Administer medications and treatments—including wound care, IV therapy, respiratory care, and specialized procedures—while maintaining accurate records.
• Monitor and interpret vital signs, lab values, and blood glucose levels, taking appropriate action when needed.
• Provide specialized care for central lines, PICC lines, and portacaths, including the administration of blood products where required.
• Assist with nutritional and hydration needs, whether through oral feeding or tube feeding, ensuring every resident’s well-being.
• Collaborate with the healthcare team to deliver individualized care plans that support both medical needs and quality of life.

Key Responsibilities

  • Conduct thorough mental health assessments to evaluate patients' conditions.
  • Develop individualized care plans in collaboration with patients, families, and healthcare teams.
  • Administer prescribed medications, monitor their effects, and educate patients on medication management.
  • Educate patients and their families about mental health conditions, treatment options, and self-care strategies.

Skills & Experience

  • Registered Nurse (RN) with a valid nursing license.
  • Specialized training or certification in mental health nursing.
  • Minimum of 2 years of experience in mental health care, preferably in a clinical setting.
  • Proficient in conducting mental health assessments, developing care plans, and administering medications.
